What to Bring to Your First Visit
To help us serve you better, we ask that you bring to your first visit the items listed below.
- Please print and complete the patient questionnaires that you can download from the Patient Profile and Registration Forms page. Completing these forms prior to your appointment will help the physician and the staff see you in a timely manner.
- Be sure to bring your insurance card.
- If you wear contact lenses or eyeglasses, please bring them with you. It is also helpful if you also bring the box your contact lenses come in. This provides us with information in regard to the brand, prescription, base curve, and diameter of the contact lenses you are currently wearing.
- If you have a prior eye problem, we recommend you obtain your previous ophthalmic records and bring them with you on the day of your visit.
